Emmi to Lead Sessions at 2016 Cleveland Clinic Patient Experience Summit
Executive Director of Patient Engagement will share insights on human-centered care transitions
Chicago, IL – May 12, 2016 – Emmi, provider of interactive patient engagement solutions, announced today that Geri Lynn Baumblatt, executive director of patient engagement, will lead two roundtable sessions at Cleveland Clinic’s Seventh Annual Patient Experience Summit. The conference attracts more than 2,100 healthcare thought leaders, hospital executives and other industry stakeholders interested in improving patient experience and outcomes.
On Monday, May 16 and Tuesday, May 17 at 3:30 p.m., Baumblatt will lead sessions titled, “Hospital to Home: Supporting Patients and Family Caregivers.” The sessions will focus on the science behind post-discharge communication, as well as best practices used to engage patients and their loved ones during care transitions.

The workshop, available to conference attendees, will provide best practices for using decision aids and train clinicians to improve SDM processes during clinical encounters.
