Rensselaer Hospital Provides Tools to Help People Better Manage Their Health

Initiative Will Support Quality and Value-Based Care Initiatives

CHICAGO — April 19, 2017 — Franciscan Health Rensselaer, part of the 14-hospital Fransciscan Health system, announced today it will provide Emmi® programs to help patients manage their health to support positive health outcomes. Emmi is a technology-enabled, interactive health education program, designed to empower people and help them manage their health. Fransican is offering Emmi as part of its ongoing commitment to deliver high-quality care and improve the health of people and communities.

Fransiscan Health Rensselaer recently became part of Fransiscan Health in September of 2015. Since 2015, the system has provided Emmi to help extend the reach and effectiveness of care teams, create greater consistency in educating and engaging patients and connect the dots throughout the continuum of care.

Patients will be prescribed Emmi programs prior to procedures or when they are scheduled for appointments. Emmi programs use simple language, compelling visuals and empathetic narration to help people make sense of complex medical information throughout their healthcare journey.The web-based programs provide understandable, actionable information and can be shared with family and friends, encouraging individuals to have meaningful conversations about their treatment with all involved in the process.
